To Register your child for a Soccer Club league, you will need:
- A Completed (and signed) registration form. Click here for a printable version of the Registration form. Forms are also available at Telluride Public Schools, Wilkinson Library, San Miguel Resource Center, and the Town Park Center.
- Physician release form, approving your child for sports participation
- 1” x 1” photo of the participating youth
- Copy of the official birth certificate
- Payment in full

The Telluride Youth Soccer Club is a not-for-profit organization. Financial assistance is available for participants who qualify. Completed applications for financial assistance for the Spring season must be received no later than March 14th.
